When you are in treatment you are focused on the fight of your life, it may be difficult to think about what lies ahead. There are physical and emotional concerns for which you need to prepare after treatment. LIVESTRONG is here to help. We created the LIVESTRONG Care Plan in partnership with OncoLink to help survivors outline their post-cancer needs. They can share and discuss this document with their oncologist and primary care teams. Below is a video talking how to create and use the LIVESTRONG Care Plan.

While you’re planning, consider writing health care and financial powers of attorney. If you have financial assets or children, you should also consider writing a will. Don’t be afraid of writing these documents. It just means you’re thinking ahead. It doesn’t mean you’re going to die of cancer.
